  5. Type your BIRTHPLACE, EMAIL, HEIGHT, WEIGHT, HAIR, EYES, MARKS, SEX, ADDRESS, TELEPHONE, EDUCATIONAL, SCHOOL, SEA SERVICE RECORDS, NBI, BSC/BT CERTIFICATES, LICENSE and ADDRESS then click SUBMIT.
  6. Print the SIRBForm that being sent to your email.
  7. Prepare the needed original document.
    1.  For New Applicant
      1. Birth Certificate NSO
      2. Valid NBI Clearance
      3. Basic Training Certificate
      4. Transcript of Records (for BS MarE/ BSMT)
    2. For SIRB Reissuance
      1. Valid NBI Clearance
      2. Basic Safety Training / Refresher / Updating
      3. License or Board Certificate, if applicable
      4. Original SIRB
      5. Sea Service Certificate
    3. For Lost or Damaged SIRB
      1. Valid NBI Clearance
      2. Basic Safety Training / Refresher / Updating
      3. License or BoardCertificate, if applicable
      4. Duly notarized affidavit of support
      5. Sea Service Certificate
  8. Additional documents to prepare:
    1. Photocopies of NBI, BST, BT Refresher, BT Updating, BT COP, All pages of SIRB, Sea Service Certificate and License
    2. White Long Folder / Documentary Stamp / White Uniform
  9. On your appointment date and time, proceed to 5th Level SM City Manila, Concepcion St. corner Arroceros Street, Manila.
  10. Pay for Documentary Stamp (P25) and if you forget to bring white long folder, it is also available for (P10) inside.
  11. Go to EVALUATION in counter 4 or 5 to submit documents and photocopies for online appointment.
  12. Go to CASHIER for payment
    1. Regular (P800) for next day release
    2. Expedite (P1,500) within 4 hours release
  13. Pay for white uniform (P25).
  14. Go to IMAGE CAPTURING area and receive the official receipt and proceed to RELEASING area if EXPEDITE processing. But if REGULAR, go back on the next day around 1pm.

Please Note: If you want to use for walk-in application of SIRB, be there at SM manila earlier than 8am considering that they can most effective receive 400 candidates in keeping with day.
